Welcome to Chalmers Physiotherapy

Chalmers Physiotherapy is obliged to provide an individually tailored rehabilitation program for optimal patient care. Proper Wheelchair and Walker Assessment

At Chalmers Physiotherapy, we provide wheelchair and walker assessment for seniors and people with long-term physical disabilities. Those who are qualified upon assessment will be able to purchase the appropriate mobility aids or wheelchair through the Assistive Device Program to assist their daily activities.


Assessment fee will be applied.


Publicly Funded Clinic-Based Physiotherapy Services

We provide physiotherapy services under the Ontario Ministry of Health publicly funded model. Your cost will be covered by the Ontario Government.


If you are:

  • Aged 65 years and older;
  • Aged 19 years and younger; or
  • Recently discharged as an in-patient of a hospital and in need of physiotherapy services that are directly related to the condition, illness, or injury for which the person was admitted to the hospital.

OR

  • Being referred by a physician or nurse practitioner based on the findings of an assessment that states the person requires physiotherapy services and is eligible for funding of services under the Ontario Disability Support (ODSP) or Ontario Works (OW) programs.
